The Ottawa Pathways to Better Health program is now accepting referrals for Community Health Workers to assist Medicare and Medicaid eligible adults residing in Ottawa County with health and social service needs with the goal of improving health outcomes and decreasing unnecessary hospitalization and emergency department visits.
